UltraScan Medical Diagnostic Imaging (UltraScan) Obligation Under PIPEDA

UltraScan is obliged under PIPEDA to protect your information which you may provide to us based on clinic-client relationship. Our diagnostic imaging clinic will keep your information confidential. 

By contacting our clinic (UltraScan) by any means, including the social medial channels, agent or service providers, you consent that we may collect, use, manage, and disclose your personal information as permitted under PIPEDA and law.

UltraScan Medical Diagnostic Imaging (UltraScan) may also collect, use, disclose your personal information in order to provide you with the required diagnostic imaging services. Furthermore, UltraScan Medical Diagnostic Imaging (UltraScan) established a number of privacy policy regulations and trained its staff in order to protect your personal information including utilizing password protection systems for stored files and client’s information, and restricting accesses to the same. 

By using our website, social media accounts, or contacting us by any other means will result in collecting your personal information; therefore, you provide UltraScan Medical Diagnostic Imaging (UltraScan) with your consent to collect your personal information. The collection of your personal information is collected and will be collected through our Client-Service Provider Relationship by lawful and fair means, and in a reasonably way. Furthermore, we may need to collect your personal information from third parties such as, but not limited to, governmental entities, Ministry of Health and Long Term Care (MOHLTC), physicians' pr prescribers' office(s), agents, private entities. The purpose for collecting such personal information is to provide you with proper and required diagnostic imaging services.

UltraScan Medical Diagnostic Imaging (UltraScan) may use your general personal information such as your email address, social medial accounts, telephone, fax number and the nature of the diagnostic imaging services that you may be interested in for marketing purposes. Additionally, we may send marketing and promotional communications to those who visited our website, social media accounts, call us, send us emails, or contacted us by any other means.  

Correctness, Currency and Completeness of Information

You must contact our clinic immediately if any of your information changes. Your information must be current, accurate, and complete at all times as it may negatively impact the outcome of our diagnostic imaging services.

Any request for additional information, beyond the summary, will be subject to additional professional legal fees and costs as it will require additional work and time from our office.

·  When we are required to provide your personal information to another entity or service provider (such as our Radiologists or staff) in order to render the required diagnostic imaging services.

·         Your consent is implied, unless you inform us in writing of otherwise; and 

·  if we hired on your behalf interpreters, translators, or expert witnesses.

We use various channel of communications such as, but not limited to, emails, social media channels, websites, and blogs. Therefore, it is important to emphasize that our channel of communications may contain links and references to other websites for third parties. Such websites are not subject to our privacy control; thus, we are not responsible for such third-party websites. Thus, we advise you to review the privacy policy of any external website or external link as may contain different privacy policy. 

Changes and Amendments of Privacy Policy

Please note that it is important to review this policy in a regular basis as we reserve the right to change or amend our Privacy Policy at any time and without providing a prior notice. We will post any changes or amendments of our Privacy Policy on our website.
